Please fill out and submit our Resale Application form to initiate your Resale Account application. Resale accounts will not be established until the fully completed Resale Account Request Form below is submitted AND we have received and verified any required documentation.

• A copy of your Sales Tax or Resale Permit
• A copy of your business license
• Photographs of your retail store (required)

Reseller Orders

Once your application is approved, you will receive an email with your reseller username and password. To place reseller orders:

• Visit our website at http://wholesale.CrystalEvolutionOnline.com
• Log in securely using your username and password
• Place your order. Once you have logged in, all items will be displayed with wholesale pricing.

Your order history will be saved in your account so that you can easily refer to it when placing new orders. Past orders remain in your account indefinitely.
